Process for Receiving Academic Accommodations
In the first few
weeks of each semester, all students seeking academic accommodations must meet
with Marni Jones, the Director of Disability Services, to receive an
accommodation letter and related forms. Each student should also schedule an
accommodations discussion meeting with the professor of each class he/she is
seeking accommodations for.
During the meeting, the student and professor
will sit down together to complete and sign the Confirmation of Accommodations
Discussion Form. On the form, the student and professor will note how
accommodations will be implemented. If receiving extended time for testing, the
student must consider any class conflicts he/she has right before or after
class, and if proctoring is needed, indicate on the form the dates and times
when tests will need to be proctored.
Once completed, the student is
responsible for returning the form to Disability Services within 3 days of the
meeting. This form must be turned in, even if the department will accommodate
the student's testing needs.
When a test proctor is needed,
Stephanie Anderberg, Program Coordinator for Accommodations, will review the
form and send out a confirmation email 1-3 days prior to each exam. If you have
questions or concerns regarding testing accommodations, or if you do not receive
confirmation of your request for test proctoring by 2 working days before an

Please note that students requesting a test proctor must give
at least 5 days notice before any exam. We will not be able to provide
proctoring for students who do not provide sufficient notice.
