The Wellness Center - Health Services will be providing flu vaccines to employees and students during a 2-day Flu Vaccine Clinic. This Wellness Event is co-sponsored by Human Resources and the Wellness Center. All participants will be required to wait 15 minutes following their injection.

Flu vaccines will be given to staff with the co-pay waived. The Wellness Center also has 200 flu vaccines available to students at no charge as long as they participate in the Flu Clinic. This year a limited number of microinjection flu vaccines will be available for those who dislike getting a traditional flu shot. You must between the ages of 18-64 to receive this vaccine.
To Register:
•Staff should register through Events Management in CLIQ via Gateway.
•Students should register through the Students Form tab in Gateway. The Wellness Center - Health Services staff cannot register students by phone or e-mail. The Wellness Center has four $25 Wal-Mart gift cards for students. All students participating in the Flu Clinic will be automatically entered in the drawing .
•Each time slot has a specific number of appointments available. If you are unable to register for a specific time, then it has been
filled and you should choose an alternate time. You may only register for one time slot.
• You should click the 2012 Flu Vaccine Clinic participation form to complete and bring along with you to your scheduled appt.
If you have a problem with the event registration, please contact the LIS Helpdesk rather than
the Wellness Center - Health Services.
Staff with questions regarding the flu vaccine should contact their health care provider.
